Indications

- Hirsutismes female major of nontumoral origin (idiopathic, syndrome of the ovaries polykystic), when they resound seriously on the psycho-emotional and social life.
- Palliative Treatment of the cancer of the prostate.

Against indications

CONTRA-INDICATE:
- Serious hepatic Affections, hepatic tumors (except metastases of a cancer of the prostate),
- tuberculosis and diseases cachectisantes,
- severe diabetes,
- existence or antecedents of thrombo-embolic affections,
- severe chronic depression,
- at the woman: those of all therapeutic gestagenes and those of the possibly associated drugs. If one uses ACETATE OF CYPROTERONE EG in the form of a combined cyclic treatment, to respect the counter-indications of oestroprogestatives associations.
- In partnership with preparations containing millepertuis (see interactions).
- Breast feeding: there do not exist data concerning the passage in the mother’s milk. Consequently, by measure of precaution, it is advisable to avoid managing this drug at the woman who nurses.
- Because of the presence of lactose, this drug is contra-indicated in the event of congenital galactosemy, of syndrome of malabsorption of glucose and galactose or of lactase deficit.
DISADVISE:
- Pregnancy: the studies carried out in the animal showed an effect feminizing amount-dependant on the acetate of cyprotérone. In private clinic, the risk is potential, in the event of strong administered doses for the period of sexual differentiation (of the 8th week of amenorrhoea until approximately 17 weeks of amenorrhoea). However, no effect of this type was reported to date, on a follow-up of a hundred pregnancies. The pregnancy does not represent an indication with the use of this product. In case of doubt about the possibility of a pregnancy, a test will be practiced before the beginning of the treatment. However, the discovery of a pregnancy during the treatment does not justify the interruption of it.
- Disadvised Associations: enzymatic inductors: anti-epileptics (phenobarbital, phénytoïne, primidone, carbamazépine), rifabutine, rifampicine, griséofulvine.

Undesirable effects

- At the man:
. Inhibition of the usually reversible spermatogenèse to the stop of the treatment (being able to involve a temporary sterility).
. Gynécomastie.
. Variation of the weight.
. Impotence.
- At the woman:
. The progestative activity of the acetate of cyprotérone can involve disorders of the rules of intercurrent the bleedings type or post-therapeutic amenorrhoeas, it makes necessary the advised posological diagram.
. Catch of weight.
- In the 2 sexes:
. Dyspnea.
. Aggravation of a venous insufficiency of the lower extremities.
. Migraines and cephalgias.
. It was brought back among patients treated by acetate amounts of cyprotérone ranging between 200-300 mg/jour, of the hepatic case of toxicity: ictères, hepatic insufficiency and of rare cases of cytolytic hepatitises of which some of fatal evolution. The majority of the reported cases concern men treated for a cancer of the prostate.
This toxicity is amount-dependant and appears several months after the beginning of the treatment. A hepatic biological assessment must be carried out before the treatment and during the treatment, in the presence of a symptomatology evoking a possible hepatotoxicity (see precautions for use).
In the event of confirmed hepatotoxicity, the treatment must be stopped, unless this one can be explained by another cause, for example the existence of metastases. In this last case, the treatment should be continued, only after strict evaluation of the benefit-risk ratio.
